Entry Forms
To compete in local Miss Texas'
Outstanding Teen
preliminary pageants, you will need to complete the 2012 Teen Contestant
Information Form, Release and Indemnity Agreement Form and the Local State
Contract 2012 ONE TIME ONLY. This single set of paperwork can be used by
each local pageant you choose to enter. In addition, contestants will need to
send copies of their completed Judges' Resume and Platform Issue Statement
to each local pageant they enter.
Complete the following three

Open this document, complete the highlighted fields. Submit electronically
to

2029 Lee St.; Mesquite, TX 75149
Release and Indemnity Agreement Form
Open this document, complete the highlighted field, and print one copy.
Use "Local/Texas" for local title
and pageant organization.
After printing, you and your parent/guardian must both sign and date. In
addition, a witness must sign and provide a phone number.
Return the signed copy by regular mail to: Tina Stringer; Miss Texas
Organization Administrative Asst.; 2029 Lee St.; Mesquite, TX 75149
Local State Contract
Open this document and complete the highlighted fields. Use the Tab key to
move from one required field to the next. Use MISS TEXAS' OUTSTANDING TEEN
LOCAL as the pageant title. Print one copy. After printing,
there are numerous places both you and your parent/guardian must sign,
initial, and date. In addition, the document must be notarized.
