Following are the required state production forms for Miss Texas 2013.

Production Form 1 - 2013 Miss Texas Duplication of Talent Presentation
Download and complete. Submit completed form by email as soon as your talent decision is made. Email completed copy to production@misstexas.org by April 1, 2013.

Following are the required forms to be completed following your local pageant.

Form 1 - Contestant Certification
Complete this form and return by regular mail to David White immediately after your local pageant is complete.

Form 2 - Scholarship Recap
Complete this form and return by regular mail to David White within 10 days following your local pageant.

Form 3 - Contestant Participation
Complete this form and return by regular mail to David White within 10 days following your local pageant.

Form 4 - Local Pageant Judges Information
Complete this form and return by email to David White at least 14 days before your local pageant.

Form 5 - Crown Receipt
Complete this form and return by regular mail to David White within 10 days following your local pageant.

Form 6 - CMN and Community Service Summary
Complete this form and return by regular mail to David White within 10 days following your local pageant.

Form 7 - Contestant Contact Information
Complete this form and return by regular mail to David White within 10 days following your local pageant.

Form 8 - Proof of Awards Paid
Complete this form and return by regular mail to David White within 30 days following your local pageant.
