Entry Forms - Miss Contestant

To compete in local Miss Texas preliminary pageants, you will need to complete the 2013 Miss Contestant Information Form, Release and Indemnity Agreement Form and the Local State Contract 2013 ONE TIME ONLY. This single set of paperwork can be used by each local pageant you choose to enter. In addition, contestants will need to send copies of their completed Judges' Resume and Platform Issue Statement to each local pageant they enter.

2013 Miss Contestant Information Form
Open this document, complete the highlighted fields, and print one copy. After printing, you must sign and date. Scan it and email it to: paperwork@misstexas.org

Release and Indemnity Agreement Form
Open this document, complete your name in the highlighted field, and print one copy. After printing, you must sign and date. In addition, a witness must sign and provide a phone number. If you are under 18, your parent must also sign and date. Scan it and email it to: paperwork@misstexas.org

Local State Contract 2013
Open this document, complete the highlighted fields. It is pre-filled for you for convenience, so ONLY enter information as needed in the blank fields. Use the Tab key to move from one required field to the next. Print one copy. After printing, there are numerous places both you and your parent/guardian must sign or initial and date, which are pages 15 through 21. In addition, the document must be notarized on page 15 and possibly 16. Return the signed and notarized copy by regular mail to: Miss Texas Paperwork, 2009 St. Pierre Drive; Carrollton, Texas 75006
